If you are attending a camp in CT, please download the necessary forms and upload them to our encrypted paperwork portal. 

1. Download and upload the CT Physical Exam Form OR upload a Physical Examination dated within 3 years of the start of camp.

2. Download and upload the Medication Administration Authorization Form

   • Only needed if the camper will be taking prescription medications while at camp.
