The College of New Jersey (TCNJ) Residential Camp

If you are attending TCNJ Residential Camp, You must submit the following documentation and then sign the TCNJ specific waiver:

1. Physical Examination dated within 18 months of the start of camp

2. Complete Immunization Records

3. The Prescription Medication Records Form

       a. Only needed if the camper will be taking prescription medications while at camp.
