Golden Goal (NY) Residential Camp

If you are attending Golden Goal Residential Camp, You must submit the following documentation and then sign the Golden Goal specific waiver:

  1. Physical Examination dated within 18 months of the start of camp

  2. Complete Immunization Records

  3. The Prescription Medication Records Form

    1. Only needed if the camper will be taking prescription medications while at camp.

After submitting the health documentation please complete the Golden Goal Specific Waiver

  • Click “Register”

  • Create an account on League Apps (if you don’t have one already)

  • Fill out the required information

  • Group Name is “Beestera Soccer 2026”
