If you are attending a camp in Massachusetts, please download the necessary forms and upload them to our encrypted paperwork portal.

1. Download and upload the MA Health & Release Form OR upload a Physical Examination dated within 18 months of the start of camp.

2. Upload a complete Immunization Record 

   • Click here to see a list of required vaccines.

3. Download and upload the Prescription Medication Records Form

   • Only needed if the camper will be taking prescription medications while at camp.
